Has Anyone Ever Had A Questionnaire Sent To You After You Have Ordered A Repeat Prescription From Your Doctor's?

Earlier today I reordered my monthly repeat prescription for my medication.
Once I'd finished writing down all of my medication and what I need them for and how much and how often I take them, I clicked on the next button and one questionnaire popped up asking about my moods (I have been diagnosed with depression but it's not because of my PCOS), which I filled in and then I got another one about my periods, which I filled in to.
I was surprised to get them as I've never had one before.

I often get the depression screening, since I've been diagnosed with that (like you, not because of PCOS), but I've never seen one about my periods. I think that's interesting.
